The Rebuilding of Hiltmarch
With the absence of divine magic for a long period, the races of the realm had to find new ways to craft basic potions, curative and stat buffing. Over time, they developed alchemical recipes in order to do this. The same knowledge translated into development of more poisons. All of this now falls under the Craft (alchemy) skill, but a survival check is necessary to find the plants necessary to create the potions. The higher level the potion/poison, the higher survival check needed to find the ingredients. Actual DCs will vary based upon terrain, and will be more of a fluid system rather than a rigid system. So, it will be up to DM discretion. You will still require an alchemists lab to create potions/poisons this way.
They can still be created with divine or arcane means.
